Requirements
- Established CVD with diabetes and hypertension
- At least one risk factor for developing HF (assessed at screening visit)
Outpatient Visit
- Clinic visits every 12 weeks
- Phone visits in between
Payment / Remuneration
- Reimbursed for travel
